When I saw the picture, I figured maybe dead-center 4 (neither wing pronounced). I thought, "Not another 4 right away... Jan wouldn't do that." So I thought a little more, and realized 6w7 was equally, if not more, likely. I fancied that she looked like a particularly anxious variant of 6w7, representative of the Intimate ("strength and beauty") and Self-preservation ("warmth") instinctual variants. The transcript of dialogue seems to support the latter hypothesis; observe Riso's comparion of 4 and 6 in _Understanding the Enneagram_. The 4's depression is most characteristically triggered by sloth in self-actualization, the 6's depression by fear of retribution. Her concerns would seem to fall almost completely into the "fear of retribution" category. I notice also, for the purposes of estimation, the patient's apparent disorientation and mental groundlessness (which may be construed as indecision) in an anxious state that is outrightly displayed, implying a 7 wing, and the expected friction of dependence / independence drives.
