If you want to sneak the Enneagram into the context, however, I would start with the self-image issues of the Feeling Triad (2, 3, and 4). Self-image, I gather, is usually the cause, so it might be 2-3-4 energy even in people who are not primarily of any of those types. As a 4 who often leans to 3, I am constantly aware of my physical image - my attention as such seems to be compulsive. I won't attempt to delve, since I lack anything close to the vast amount of knowledge available on anorexia and bulemia. As a physician with knowledge of the Enneagram, you might find it best to examine the personality fixations, in whatever way possible or appropriate, of your own patients.
